Living with a neurological condition or brain injury can affect many areas of everyday life, from mobility and communication to memory, behaviour and emotional wellbeing. Our neurological care services provide consistent, specialist support that helps individuals live safely and confidently at home.

 

We work closely with families, commissioners and healthcare professionals to create personalised care plans that respond to each person’s needs, routines and long-term goals.

Who this service supports

Our clinical care services are designed for individuals who require ongoing medical interventions in the home. This often includes people whose needs are too complex for standard domiciliary care and who depend on trained staff to safely deliver clinical support as part of their daily routine.

​

Many of the individuals we support require structured care plans aligned with NHS or clinical guidance, particularly where higher levels of medical risk are involved. Clinical care is frequently delivered alongside other forms of complex care, including neurological support or physical disability services.

What clinical care may include

​

Clinical support is always tailored to the individual and delivered in line with their care plan. Depending on the person’s needs, this may include tracheostomy care, ventilation support and respiratory management, as well as PEG and enteral feeding.

​

Our teams also support with catheter and stoma care, medication administration and ongoing medication management. Where required, staff can assist with wound care, pressure sore management and other clinical monitoring that forms part of the individual’s treatment plan.

​

All clinical tasks are carried out by appropriately trained staff. Competency is assessed, monitored and regularly reviewed to ensure the highest standards of safety and care.

How clinical care is delivered

Clinical care is delivered within a clear governance framework that prioritises safety, accountability and consistency. Care planning is informed by clinical guidance and, where needed, input from multidisciplinary healthcare teams.

​

Staff are trained specifically to meet the clinical needs of each individual, ensuring they are confident and competent in the tasks required. Continuity of staff is a key priority, helping build familiarity and trust while reducing the risk of error.

Care packages are reviewed regularly, with clear escalation pathways in place to respond quickly if needs change or risks increase.

Commissioning and care transitions

​

Clinical care is most often commissioned as a long-term service for individuals with ongoing medical needs. We also support people during key transition periods, including moving from hospital or rehabilitation settings back into the community.

​

In addition, we can provide step-up or step-down clinical care packages and support situations where urgent care is required following a provider breakdown.

Where immediate support is needed, interim arrangements can often be mobilised quickly, depending on location.

 

Longer-term packages are planned carefully to ensure the right staff are recruited, trained and embedded within the care team.
