Who this service is for

Our clinical care service supports individuals who:

  • Require ongoing clinical interventions delivered in the home

  • Have complex medical needs that cannot be met through standard domiciliary care

  • Depend on trained staff for safe and consistent clinical support

  • Require structured care planning aligned with NHS or clinical guidance

  • Present with higher levels of clinical risk requiring robust governance
     

Clinical care is often delivered as part of a wider complex care package and may overlap with physical disability or neurological support.
 

What clinical care typically includes

Clinical care is tailored to the individual and may include:

  • Tracheostomy care and ventilation support

  • PEG and enteral feeding

  • Catheter and stoma care

  • Medication administration and management

  • Wound care and pressure sore management

  • Respiratory care and suctioning

  • Care planning in line with clinical and NHS guidelines
     

All clinical tasks are delivered by appropriately trained staff, with competency assessed and reviewed on an ongoing basis.
 

How care is delivered

Clinical care is delivered within a clear governance framework designed to support safety, consistency and accountability.


Care planning is informed by clinical guidance and multidisciplinary input where required. Support is delivered by staff trained specifically for the individual’s clinical needs, with regular reviews and escalation pathways in place to manage risk and respond to change.
 

Continuity of staff is prioritised to ensure familiarity, reduce error and support consistent clinical outcomes.
 

Commissioning and transitions

Clinical care is most commonly commissioned as a long-term service. We also support:

  • Step-up and step-down clinical care packages

  • Transitions from hospital or rehabilitation settings

  • Interim or emergency clinical support following provider breakdown
     

Where urgent support is required, interim arrangements can be mobilised quickly depending on location, while longer-term packages are planned to ensure appropriate training and continuity.
