Who we support
Our neurological care services are designed for individuals living with acquired or progressive neurological conditions. This includes people who have experienced an acquired brain injury or traumatic brain injury, as well as those living with conditions that affect cognitive function, movement or communication.

Many individuals we support benefit from structured routines and consistent care teams, particularly where neurological conditions impact memory, executive function or emotional regulation. In many cases, neurological support forms part of a wider complex care package that may also include physical disability or clinical care.

Neurological care is always shaped around the individual, but commonly includes support for:
Acquired brain injury and traumatic brain injury
Cognitive impairment, memory loss and executive functioning challenges
Progressive neurological conditions such as multiple sclerosis, Parkinson’s disease, motor neurone disease and cerebral palsy

Transitions and ongoing support

Neurological care is often commissioned as a long-term service, particularly for individuals living with the lasting effects of brain injury or progressive neurological conditions.

We also support individuals during important transitions, including moving from rehabilitation units back into the community, step-up or step-down care packages, and situations where urgent support is required following a provider breakdown.

When new care packages are introduced, mobilisation is carefully planned to ensure staff are trained, supported and fully embedded. Where immediate care is needed, interim arrangements can be put in place while long-term teams are established.
